Yes, but not on every swim stop and not every week. The Mediterranean gets seasonal blooms of the mauve stinger, a small jellyfish that shows up more in warm, calm patches, typically later in the season. It is not something that should change when you book.
The crew checks the water before you swim off the back of the yacht, and if jellyfish are around, they simply move the boat to a different bay or anchorage rather than let anyone swim through them. It is a five-minute delay, not a ruined afternoon.
If someone is stung, every yacht carries a first aid kit for it: rinse with seawater, apply vinegar if it is on board, and treat it like any minor sting. It is uncomfortable, not dangerous, for the vast majority of people.
